If you answered Yes to either question, Association Management may request further documentation for ESA's or may ask further questions for SA's allowable under FHA rules for COA's.
Section 5 — Association Registration
Pets must be registered with the Association Manager no later than two (2) days after arrival, or the first business day the Manager's office is open. Submit this completed form together with a recent photograph of each pet to the Manager's office or via the Association website.
Section 6 — Owner Acknowledgement & Signature

The undersigned unit owner certifies that all information provided is accurate and that all pets comply with the Association's Declaration of Condominium, By-Laws, and Rules & Regulations, including size limits, immunization, leash, and conduct requirements. The owner accepts full responsibility for any damage or injury caused by their pet(s). Violations may result in fines up to $100.00 per diem per Florida Statute §718.303, removal of pet(s), and/or other available remedies.
